The Tunisia Health Declaration Form is an electronic travel screening required for all foreign tourists who wish to enter Tunisia. This document is required to help at least prevent the spread of Middle East respiratory Syndrome (or MRS), a deadly disease that has swept through Europe and the Middle East. This is essentially a two page form that requests basic information regarding your health history.
